Overview

The Social Worker at ConcertoCare is a core interdisciplinary team member responsible for providing integrated medical, behavioral, and social care to patients with complex health care needs. This role involves working with a multidisciplinary team to support patients' mental and physical health.

• 100% remote position
• Full-time role, Monday - Friday, 8am-5pm PST
• Requires Washington State LICSW
• Supports behavioral health needs of patients

Requirements

• Master’s degree in Social Work
• Current Washington State LICSW required
• 2+ years of healthcare/behavioral health experience
• Strong problem-solving and organizational skills
• Ability to manage patient complexity
• Effective communication skills
• Proficient in Microsoft Office and EMR systems
• Reliable internet connectivity required
• Professionalism and teamwork skills
• Geriatric care experience is a plus
