Requirements

• Master’s degree in Social Work.
• Current New York LCSW required.
• 2+ years of prior healthcare/behavioral health experience.
• Strong problem-solving and organizational skills.
• Geriatric care experience highly desired.
• Ability to manage patient complexity and diverse needs.
• Effective written and verbal communication skills.
• Proficient in Microsoft Office and EMR documentation.
• Reliable internet connectivity required.
• High level of professionalism.
