Social Worker - LCSW - Remote

Overview

This position is fully remote and must reside in New York.

In Short

• Supports the behavioral health needs of patients on an assigned patient caseload.
• Uses evidence-based models of care to deliver brief treatment for common mental health issues.
• Provides consultation to group residences to ensure facility staff can recognize changes in patients' behavioral health.
• Supports psychotropic medication management by collaborating with prescribing providers.
• Participates in regularly scheduled caseload reviews and clinical team meetings.
• Completes and documents assessment of patients in a timely manner.
• Facilitates treatment plan changes for patients not demonstrating improvement.
• Serves as a subject matter expert in behavioral health care.
• Ensures care is delivered in a manner that honors patient’s diverse values and needs.
• Documents patient encounters and referral activities in electronic health record.

Requirements

• Master’s degree in Social Work.
• Current New York LCSW required.
• 2+ years of prior healthcare / behavioral health experience required.
• Strong problem-solving and organizational skills.
• Geriatric care experience highly desired.
• Ability to manage patient complexity and multiple clients.
• Effective communication skills in writing and verbally.
• Ability to work in a fast-paced, dynamic environment.
• Proficient in Microsoft Office Suite and EMR experience.
• Reliable internet connectivity required.
