Lead Field Nurse (LPN) - Remote

Overview

The Lead Field Nurse (LPN) is responsible for mentoring and coaching field staff while managing a panel of patients through routine and urgent home visits and care coordination activities.

Requirements

• Active LPN, LVN, or RN license.
• At least 2 years of direct patient care experience.
• IV or blood draw certification.
• Current certification in Basic Life Support (BLS).
• Experience providing in-home clinical care.
• Proficiency in various clinical procedures.
• Ability to conduct independent home visits.
• Willingness to travel 75%+ of the time.
• Strong collaboration and communication skills.
