Overview

The Field Nurse (LVN) provides clinical care to patients in their homes and offers virtual clinical support, ensuring patients receive appropriate care in a timely manner.

In Short

• Deliver direct nursing care through in-home and virtual visits.
• Facilitate virtual clinical visits under supervision.
• Provide support for clinical visits, including documentation and care coordination.

Requirements

• Active LVN license in California.
• 2+ years of clinical experience in urgent care or home health.
• Certification in Basic Life Support (BLS).
• Ability to conduct independent home visits.
• Travel 75%+ within an assigned area.
• Prior experience with aging adults preferred.
• Strong computer and software skills.
• Valid driver's license and clean driving record.
• Ability to work flexible hours as needed.
