Overview

The Housing Stability Case Manager is responsible for working with families and individuals in need of healthcare, housing, and community support services, aiming to improve health and end homelessness in Washington, DC.

In Short

• Work with families to develop and update case management plans.
• Meet regularly with families to review progress and follow-up on tasks.
• Assist families with housing search and placement.
• Network with community resources and make referrals.
• Complete required assessments with families.
• Collaborate with Youth Specialists regarding children's needs.
• Communicate with treatment programs and vocational programs.

Requirements

• Bachelor’s degree in social work or related field required.
• Minimum one year experience in social service preferred.
• Experience with individuals or families experiencing homelessness preferred.
• Strong interpersonal and organizational skills required.
• Valid driver’s license and access to a car required.
• Hybrid work model with remote and on-site expectations.
