Overview

The Asthma Counseling Specialist will develop and implement care plans to help participants achieve good asthma control, monitor outcomes, and provide case management services.

Requirements

  • Experience in case management and community outreach.
  • Strong communication and organizational skills.
  • Ability to work with diverse populations.
  • Knowledge of asthma management and education.
  • Proficient in data entry and program evaluation.
