Care Coordinator - D-SNP (Temporary) - Remote

Overview

This is a temporary position for a Care Coordinator focusing on the Medicare Dual Special Needs Plan (D-SNP), responsible for coordinating services for members and collaborating with partners to improve care transitions.

Requirements

• Knowledge of health care principles and practices.
• Experience in customer service.
• Understanding of the diverse needs of D-SNP populations.
• Ability to educate on care coordination.
• Strong documentation and organizational skills.
• High school diploma or equivalent required.
• Three years of experience in a health care setting preferred.
• Equivalent combination of education and experience may qualify.
