Overview

The Senior Care Manager is responsible for developing, assessing, planning, implementing, and coordinating complex care management activities focused on behavioral health needs to provide quality, cost-effective healthcare outcomes for members.

In Short

  • Develop and coordinate personalized care plans based on member needs.
  • Educate members and families on available services and benefits.
  • Conduct telephonic, digital, and in-home visits for assessments.
  • Monitor member status and revise care plans as necessary.
  • Collaborate with healthcare providers to ensure timely access to care.
  • Maintain compliance with state and federal regulations.
  • Provide guidance and support to new clinical hires.
  • Engage in continuous improvement of care delivery.
  • Manage a caseload of complex/specialty cases.
  • Participate in training and onboarding processes for new staff.

Requirements

  • Master's degree in Behavioral Health or Social Work, or Nursing.
  • 4-6 years of related experience in care management.
  • Licensed Behavioral Health Professional or RN required.
  • Experience with behavioral health and social determinants of health.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of healthcare regulations and compliance.
  • Experience in patient education and support.
  • Proficient in using telehealth and digital communication tools.
