Licensed Clinical Social Worker - Remote

Become a part of our caring community and help us put health first. The LCSW serves as a core member of the Care Integration Team, supporting complex, high-risk patients through proactive psychosocial assessment, short-term interventions, and connection to community-based resources.

In Short

• Conduct comprehensive assessment of behavioral health needs.
• Employ various strategies to address acute behavioral health symptoms.
• Identify barriers to effective patient engagement.
• Monitor patient progression towards desired outcomes.
• Make recommendations to providers about external resources.
• Participate in care planning with a multidisciplinary team.
• Provide support for advance care planning discussions.
• Assist with suicide risk assessment and management.
• Cover for other LCSWs as needed.
• Adhere to recommended visits per day.

Requirements

• Master’s degree in social work (MSW).
• Licensed Clinical Social Work License (LCSW) in Texas.
• Minimum of 4 years’ experience as a Clinical Social Worker.
• Knowledge of community health and behavioral health resources.
• Familiarity with electronic medical record (EMR) documentation.
• Bilingual English/Spanish preferred.
• Experience with the geriatric population preferred.
• Experience in crisis management and intervention.
• Familiarity with the South Texas area preferred.
