In Short

• Collaborate with treatment team members to provide quality care in an outpatient setting.
• Conduct assessments and formulate treatment plans under supervision.
• Meet with patients for individual sessions based on treatment progress.
• Assist clients in aftercare and discharge planning.
• Provide case management services and referrals as needed.
• Maintain accurate documentation in the Electronic Health Record (EHR).
• Participate in treatment team meetings and staff development.
• Communicate with medical professionals to ensure timely clinical services.
• Comply with confidentiality laws and demonstrate cultural sensitivity.
• Abide by PAX Health's code of ethics.

Requirements

• Licensed Clinical Social Worker licensed in New York State.
• Bilingual (Spanish/English) is a plus.
• Authorization number from New York State Workers' Compensation Board.
• Ability to work as part of a community mental health team.
• Familiarity with community support systems and healthcare resources.
• Good organizational and administrative skills.
• Flexibility in scheduling, including weekends.
• Knowledge of professional ethics and state mental health code.
• Valid driver's license and access to a private vehicle.
