Lead Care Navigator - Remote

Overview

This role offers the opportunity to be the first clinical hire at a growing care navigation program, where you will manage an active patient caseload while helping to design workflows and standards for care delivery.

Requirements

• LVN preferred; RN considered with leadership potential.
• 2–5 years of care navigation or related experience.
• Fluency in SDOH and connecting patients to resources.
• Builder mentality and adaptability in fast-changing environments.
• Comfortable with care management platforms and EHRs.
• Resilient problem-solver with low ego and high output.
