Clinical Program Manager, Care Navigation - Remote

Requirements

• Active multi-state Nurse Licensure Compact (NLC) license.
• Preferred LVN; RN considered with leadership potential.
• Experience in care navigation or related patient-facing role.
• Telehealth experience and ability to connect with patients remotely.
• Fluency in social determinants of health.
• Strong problem-solving and adaptability skills.
• Comfortable with care management platforms and EHRs.
• Experience in a startup or pilot program is a plus.
• Bilingual skills are advantageous.
