Overview

The Transitional Care Associate plays a critical role in facilitating the safe and timely transition of patients from acute care settings to alternative levels of care, ensuring effective discharge planning and coordination.

• Conduct discharge planning for patients transitioning from hospital to post-acute care.
• Facilitate insurance prior authorization and hospital care coordination.

Requirements

• Bachelor’s degree in social work or related field, or relevant nursing license.
• Knowledge of community resources and healthcare services.
• Effective communication and customer service skills.
• Time management skills and ability to work under pressure.
• BLS certification required.
• Previous experience in healthcare or social services preferred.
• Ability to work flexible hours.
• Arizona Fingerprint Clearance Card may be required for specific roles.
