In Short

• Provide transformative care to patients as a Peer Specialist.
• Engage with patients via phone and/or video.
• Serve as a role model, mentor, advocate, and motivator to recovery.
• Conduct outreach to patients for encouragement and support.
• Document patient interactions in medical records.
• Coordinate care with the patient care team.
• Escalate urgent matters to care team providers.
• Conduct daily telephonic outreach based on patient needs.
• Work collaboratively with internal and external care team members.
• Help patients check-in on goal progress and problem solve.

Requirements

• First-hand experience with a mental health diagnosis and/or addiction.
• Strong communication skills.
• Ability to work in a team environment.
• Empathy and understanding of patient experiences.
• Skills in advocacy and motivation for recovery.
• Experience in healthcare or patient support is a plus.
• Organizational skills for documenting patient interactions.
• Ability to coordinate with various care team members.
• Willingness to conduct outreach via phone and video.
