Overview

The Nurse Navigator is responsible for triaging new patient appointments, scheduling initial visits, and providing support throughout the care continuum. This role involves collaborating with a multidisciplinary team to ensure effective patient care and education.

Requirements

• Experience in patient education and support.
• Strong organizational skills.
• Ability to work independently and as part of a team.
