Overview

As a Remote Oncology Nurse Navigator at Thyme Care, you'll play a crucial role in supporting cancer patients and their families, providing clinical assessments, education, and care coordination throughout their cancer journey.

In Short

  • Provide triage, support, and education to cancer patients.
  • Conduct clinical assessments and manage member health.
  • Facilitate end-of-life care discussions as needed.
  • Engage with members through phone, email, and video communication.
  • Collaborate with healthcare teams to ensure quality care.
  • Support members in navigating their oncology care continuum.
  • Participate in case conferences to monitor member progress.
  • Address social determinants of health impacting patient care.
  • Be available for urgent clinical escalations.
  • Perform other duties as assigned related to department needs.

Requirements

  • Bachelor of Science in Nursing and RN license required.
  • 5 years of nursing experience, with 3 years in oncology preferred.
  • Oncology-related certifications required or to be obtained within 2 years.
  • Strong communication and organizational skills.
  • Ability to multitask in a fast-paced environment.
  • Proactive and action-oriented mindset.
  • Empathy and a member-centric approach.
  • Adaptability to change and ambiguity.
