Overview

XstremeMD is seeking a detail-oriented and clinically strong LPN Case Manager to join our growing team. This is a remote, telephonic role focused on coordinating care, supporting patients, and improving health outcomes — all without direct patient contact.

In Short

  • Conduct telephonic case management and patient follow-up
  • Perform clinical assessments and identify patient needs
  • Document all interactions accurately in EMR systems
  • Collaborate with internal clinical teams to support care plans
  • Ensure timely and appropriate case resolution
  • Maintain high standards of customer service and patient satisfaction
  • Manage caseload efficiently while meeting productivity expectations

Requirements

  • Active LPN compact license
  • Strong critical care assessment skills
  • Proficiency with Electronic Medical Records (EMR)
  • Proficiency with Microsoft Office Suite (Word, Excel, Outlook)
  • Strong computer literacy and ability to navigate multiple systems
  • Excellent communication and critical thinking skills
