LPN Case Manager - Remote

Overview

We are seeking a motivated and detail-oriented LPN Case Manager to join our remote team at XstremeMD, specializing in delivering exceptional medical case management services in high-risk industries.

In Short

• Coordinate medical care for injured or ill individuals.
• Act as a liaison among care providers, insurers, employers, and employees.
• Monitor case progress and report updates.
• Support cost-effective care strategies while maintaining quality outcomes.
• Complete initial follow-up assessments within 24 hours.
• Maintain accurate records and meet performance benchmarks.
• Stay updated on clinical best practices.
• Work-from-home position with a professional environment.
• Flexibility for occasional evening, night, weekend, or holiday hours may be needed.
• Participation in monthly staff meetings and training required.

Requirements

• Current, valid LPN licensure (multi-state licensure preferred).
• Minimum 2 years clinical nursing experience required.
• ER or ICU experience preferred.
• Previous case management and workers’ compensation experience is a plus.
• Strong organizational, communication, and interpersonal skills.
• Proficient in Microsoft Office and typing (minimum 50 wpm).
• Ability to work independently and manage priorities effectively.
• Must pass background check and drug screening as applicable.
