XstremeMD is seeking an experienced and highly skilled Registered Nurse Case Manager to provide telephonic case management and clinical support. This role is part of a team that delivers 24/7 medical coverage, requiring flexibility in scheduling while striving to maintain strong work-life balance.

• Perform telephonic triage, assessment, and case management.
• Coordinate and monitor patient care plans.
• Serve as liaison between patients, providers, employers, and carriers.
• Ensure timely documentation in EMR systems.
• Support high-quality, cost-effective care and return-to-work outcomes.
• Maintain consistent communication and follow-up on assigned cases.
• Meet productivity and quality benchmarks.

Requirements

• Active Registered Nurse (RN) license (multi-state/compact preferred).
• Minimum 2+ years of clinical nursing experience.
• Strong critical care assessment skills.
• Proficiency in EMR systems and Microsoft Office Suite.
• Strong computer literacy and ability to multitask across systems.
• Excellent communication, organization, and critical thinking skills.
