VNA RN Case Manager - Remote

Overview

The VNA RN Case Manager is responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.

In Short

• Responsible for providing skilled nursing care and case management.
• Works in accordance with the State Nurse Practice Act.
• Utilizes nursing theory and processes for patient care.
• Communicates and documents observations and assessments.
• Promotes and maintains the health of individuals through direct care.
• Reports to the VNA Clinical Team Manager.
• Requires a nursing degree and current RN license in Massachusetts.
• Minimum of one year of medical/surgical nursing experience required.
• Home care/hospice experience preferred.
• Requires a valid driver’s license and reliable transportation.

Requirements

• Graduate of an approved, accredited school of professional nursing required; BSN preferred.
• Current license to practice as a Registered Nurse in Massachusetts required.
• Current CPR certification required.
• Minimum of one year medical/surgical nursing experience required.
• Demonstrated excellent verbal and written communication skills required.
• Strong interpersonal skills required.
• Demonstrated organizational and time management skills required.
• Current driver’s license and good driving record required.
