VNA RN Case Manager - Remote

Overview

The VNA RN Case Manager is responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.

In Short

• Provide skilled nursing care and case management.
• Utilize nursing theory and processes.
• Communicate and document observations and assessments.
• Promote and maintain health through direct care and counseling.
• Work under the supervision of the VNA Clinical Team Manager.
• Requires a current RN license in Massachusetts.
• Minimum of one year of Medical/Surgical nursing experience required.
• Home care/hospice experience preferred.
• Must have a valid driver's license and reliable transportation.
• Part-time, per diem position with hybrid work location.

Requirements

• Graduate of an accredited nursing school; BSN preferred.
• Current RN license in Massachusetts.
• CPR certification required.
• Excellent verbal and written communication skills.
• Strong organizational and time management skills.
• Driver’s license with a good driving record.
