Overview

The RN Case Manager plays a crucial role in coordinating healthcare services for patients, ensuring they receive the necessary support and referrals both within and outside the clinic.

• Join the Care Compass Clinical Team in the East Region.
• Work as a per diem RN Case Manager with day shifts.
• Coordinate patient care and referrals to external services.

• Compensation ranges from $28.20 to $68.49 per hour.

Requirements

• Associate's Degree in Nursing required; Bachelor's preferred.
• Registered Nurse (RN) License for Massachusetts required.
• 2+ years of case management or related experience preferred.
• Proficient in Microsoft Office and healthcare software.
• Strong communication and problem-solving skills.
• Ability to maintain confidentiality of client and staff information.
• Understanding of healthcare and managed care systems preferred.
