Medical Social Worker - Remote

Overview

The Medical Social Worker is a key member of a clinic, contributing to multiple care teams to support the social service needs of our patients, helping them maintain or improve their health status and avoid hospitalizations.

In Short

• Support high-need patients through telephonic check-ins and clinic visits.
• Partner with patients to identify goals and reduce barriers to care.
• Facilitate patient referrals to community agencies and resources.
• Navigate Managed Care plans for mindful utilization.
• Conduct biopsychosocial assessments using evidence-based tools.
• Participate in care team meetings to discuss patient caseloads.
• Complete case and progress documentation in a timely manner.
• Must have a Master’s degree in Social Work.
• Minimally licensed in the state of practice.
• Prior experience in clinical social work or case management preferred.

Requirements

• Master’s degree in Social Work required.
• Minimally licensed in the State of Practice.
• Minimum one year of clinical social work experience.
• Experience with independent care management or case coordination.
• Experience conducting home visits and providing telephonic support.
• Experience working on a multidisciplinary team preferred.
• Motivated and flexible attitude.
• US work authorization required.
