Social Worker - Remote

Overview

This job collaborates with all disciplines for assessing, planning, implementing and evaluating psychosocial factors which contribute to patients' illnesses and/or have an impact on continuity of care.

In Short

• Completes and documents psychosocial assessments and intervention.
• Provides crisis intervention/counseling to enhance patient/family coping mechanisms.
• Engages the patient/family and interdisciplinary team in the discharge process.
• Provides education regarding psychosocial factors related to illness.
• Participates to enhance the role of social work within the hospital.
• Performs other duties as assigned or required.
• Requires a Master’s degree in Social Work.
• Background clearances may be required depending on location.
• Preferred LSW or LCSW eligibility.
• 2 years’ experience in a hospital/multidisciplinary health care setting.

Requirements

• Master’s degree in Social Work.
• Older Adults Protective Services Act clearance may be required.
• Act 34 Criminal Background Clearance Certificate.
• Act 33 Child Abuse Clearance Certificate.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
• 2 years’ experience in a hospital/multidisciplinary health care setting.
