Requirements

• Assess Needs: Evaluate the needs of individuals, families, or communities to identify necessary resources and services.
• Create Care Plans: Develop care plans to address clients’ needs and goals.
• Connect with Resources: Link clients with appropriate community resources such as food stamps, childcare, healthcare, and mental health services.
• Follow-Up: Ensure clients’ needs are met and evaluate the effectiveness of service plans.
• Advocate: Assist clients in navigating public assistance and social service systems.
• Community Outreach: Organize and promote community outreach programs and workshops.
