Overview

As a Community Health Advocate at CVS Health, you will work closely with high-risk patients to improve their health and connect them with essential resources, all while being part of a compassionate and dedicated team.

In Short

  • Work one-on-one with high-risk patients.
  • Conduct needs assessments to understand patients' health goals.
  • Provide emotional support and connect patients to community resources.
  • Make weekly follow-up contacts with patients.
  • Coordinate with clinical care teams for patient information.
  • Co-facilitate support groups for patients.
  • Document interactions thoroughly and accurately.
  • Manage a caseload and achieve patient care benchmarks.
  • Utilize motivational interviewing techniques.
  • Collaborate with other Community Health Workers.

Requirements

  • High School Diploma required.
  • Long-time resident of assigned location with knowledge of community resources.
  • Comfortable with home visits and outreach.
  • Prior experience as an outreach worker is a plus.
  • Ability to exercise good judgment with patients and care teams.
  • Excellent oral and written communication skills.
  • Exceptional active listening skills.
  • Proficient in MS Word and data entry.
  • US work authorization required.
  • Must embody the values of positivity and determination.
