Case Manager 2 - Health Home - Remote

Overview

This position promotes the ability of high-risk individuals to achieve independent living skills and stability in their psychiatric or medical conditions through case management.

In Short

• Full-time position with a schedule of Monday-Friday 8:00am-4:30pm.
• 3 days a week remote work after 90 days.
• Provide one-on-one support to service recipients.
• Coordinate and monitor the provision of individualized services.
• Develop care management plans based on assessed strengths and needs.
• Advocate for individuals to access needed services.
• Maintain written progress notes according to agency guidelines.
• Assist program participants with various supports including academic and health.
• Ensure compliance with federal, state, and local regulations.
• Perform other duties as assigned.

Requirements

• Bachelor's degree with two years of relevant experience or a licensed RN with two years of experience.
• Master's degree with one year of relevant experience preferred.
• Must maintain a valid driver’s license and acceptable driving record.
