Health Home Plus Care Manager - Remote

Overview

The Health Home Plus Care Manager is responsible for managing a caseload of clients with Serious Mental Illness, providing assessments, developing care plans, and facilitating access to necessary services. This role requires both in-person and telephonic communication with clients to ensure they receive adequate support and resources.

• Manage a caseload of up to 20 clients.
• Conduct assessments and develop individualized care plans.

Requirements

• Master's or Bachelor's degree in a relevant field.
• Experience working with clients with Serious Mental Illness.
• Strong communication skills for interacting with clients.
• Ability to manage competing priorities effectively.
• Proficient in Microsoft Office.
• Experience in conducting care conferences.
