Overview

The Care Manager is responsible for the assessment, care planning, and coordination of care and services for members with behavioral health conditions, ensuring a person-centered approach to care management.

In Short

  • Conduct assessments and develop individualized care plans.
  • Coordinate care and monitor member progress.
  • Provide education to members and families on health strategies.
  • Oversee assigned care support team activities.
  • Conduct health risk assessments and crisis interventions.
  • Work with interdisciplinary teams to improve care outcomes.
  • Ensure compliance with care management standards.
  • Advocate for members' care needs and address gaps in care.
  • Utilize knowledge of mental health and substance abuse resources.
  • Maintain strong communication with all stakeholders involved in care.

Requirements

  • Current licensure in relevant fields (LCSW, LPC, RN, etc.).
  • Minimum of 5 years of experience in healthcare or behavioral health.
  • Strong organizational and time management skills.
  • Experience with care management processes.
  • Knowledge of utilization management procedures.
  • Ability to analyze and implement solutions for quality care.
  • Understanding of DSM V and psychiatric/medical terminology.
  • Excellent written and verbal communication skills.
