Overview

As a Registered Nurse Case Manager at 21st Century Home Health Services, you will utilize your clinical skills to create and implement care plans for patients in their homes, ensuring their wellness and preventing re-hospitalization.

Requirements

  • Current and unrestricted CA RN license.
  • Current BLS/CPR Certification.
  • Valid CA Driver's License.
  • High level of clinical knowledge in home health care preferred.
  • Experience with interdisciplinary teams desired.
  • Case Management fundamentals highly desired.
  • Experience with IV medication infusions preferred.
  • Self-directed and empathetic.
  • Outstanding interpersonal and leadership skills.
  • Organized and critical thinker.
