The Part-Time Admission Nurse at 21st Century Home Health Services is responsible for conducting initial assessments of patients in the Berkeley area, ensuring high-quality home care and collaborating with a clinical team.

Requirements

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience with OASIS assessments.
• Clinical knowledge in home health care preferred.
• Experience with high acuity patients is a plus.
• Strong interpersonal and leadership skills.
• Self-directed and organized.
