In Short

• Hybrid – Remote position with occasional travel.
• Conduct Comprehensive Health Evaluations for CKD and ESKD patients.
• Manage patient care and disease progression.

• Full-time schedule, Monday–Friday, no nights or weekends.

Requirements

• Passion for caring for patients with chronic illnesses.
• Willingness to adapt as the program evolves.
• Minimum 2 years’ experience as a Nurse Practitioner.
• Current, unrestricted NP license in state of practice.
• DEA license or eligibility within 90 days.
• Current CPR/BLS certification.
• Valid driver’s license and insurance.
• Strong clinical judgment and decision-making skills.
• Experience in Value-Based Care preferred.
• Experience with Cerner or similar EHR systems preferred.
