Bilingual Case Manager (Remote Flexible, Spanish Speaking)

Overview

The Bilingual Case Manager at Pair Team will play a critical role in providing compassionate care and support to individuals with complex health needs, while primarily working remotely.

• Full-time position focused on case management.
• Remote work with occasional on-site visits in Sacramento.
• Engage with individuals living with Serious Mental Illness/Substance Use Disorder.
• Utilize evidence-based approaches to promote health goals.
• Conduct telephonic and SMS outreach for follow-ups.
• Collaborate with multidisciplinary teams for comprehensive care.
• Support members in navigating social support systems.
• Maintain compliance with health plan regulations.
• Identify barriers to care and assist in overcoming them.
• Provide technical support using online collaboration platforms.

Requirements

• 1+ years of general work experience, preferably in case management.
• Bilingual in English and Spanish.
• Strong understanding of cultural fluency.
• High degree of empathy and organizational skills.
• Ability to work collaboratively in a multidisciplinary team.
• Strong technical skills, including CRM databases and basic software.
• Valid driver’s license and auto liability insurance.
