Foundation Health Coach - Remote

Overview

The aim of a Foundation Health Coach is to work as part of our Prevention Coaching team to deliver excellent patient care across our National Diabetes Prevention Programme (NDPP), Diabetes Structured Education (DSE) programme and our Tier 2 Weight Management programme.

In Short

• Provide high quality, evidenced based clinical care.
• Adopt a patient-centred approach to care.
• Meet internal and external KPIs.
• Overcome remote working barriers.
• Share feedback with Clinical Services Manager.
• Attend monthly clinical supervision and training sessions.
• Deliver coaching remotely via phone or app.
• Utilise diet and nutrition knowledge.
• Maintain accurate clinical records.
• Manage clinical risk incidents.

Requirements

• Expertise in diet and nutrition.
• Experience in behavioural change techniques.
• Ability to handle sensitive data.
• Knowledge of Information Governance regulations.
• Autonomous working capability.
