Registered Nurse, Integrated Care Manager - Remote

Overview

As a Registered Nurse Integrated Care Manager at CVS Health, you will coordinate care for Oak Street Health members, working collaboratively with interdisciplinary care teams to ensure optimal health outcomes.

In Short

• Manage an assigned caseload of members.
• Create and nurture relationships with members for their health and well-being.
• Conduct assessments and develop care plans.
• Document visits in the electronic health record.
• Maintain HIPAA standards and confidentiality.
• Work collaboratively with care teams including PCPs and specialists.
• Travel daily to meet members in various locations.
• Utilize Microsoft Office and Google Suite for documentation.
• Participate in interdisciplinary care team meetings.
• Provide patient-centered care to Medicare patients.

Requirements

• Current RN License in good standing.
• Minimum of three years clinical experience.
• Experience in Care Coordination/Case Management preferred.
• Knowledge of community resources and clinical standards of care.
• Strong verbal and written communication skills.
• Ability to work autonomously and manage multiple priorities.
• Experience with Electronic Medical Records platforms preferred.
• Access to reliable transportation.
• Customer-focused interpersonal skills.
• Problem-solving skills and a positive attitude.
