Integrated Nurse Case Manager - Remote

Overview

Sentara Healthcare is currently hiring an Integrated Nurse Case Manager for Roanoke, VA location! This is a Full Time position with day shift hours and great benefits!

In Short

• Responsible for case management services within the scope of licensure.
• Develops, monitors, evaluates, and revises the member's care plan.
• Performs telephonic or face-to-face clinical assessments.
• Identifies high-risk members and coordinates care.
• Manages chronic illnesses and ensures efficient utilization of health benefits.
• Facilitates authorizations/referrals within benefits structure.
• Interfaces with Medical Directors and Inter-Disciplinary Teams.
• Presents cases at case conferences.
• Ensures compliance with regulatory and company policies.
• May assist in problem solving with provider issues.

Requirements

• Education: Associates or Bachelors Degree in Nursing.
• Certification/Licensure: Registered Nurse License (RN) required.
• Experience: 3 years in Nursing, discharge planning preferred.
• Managed Care experience preferred.
• Private duty nursing experience preferred.
