Nurse Case Manager - NICU - Remote

Overview

The Nurse Case Manager will focus on empowering the members’ family to support optimal wellness while advocating for the necessary medical services across the health care continuum.

• Full-time position for a Nurse Case Manager in NICU.
• Must reside in South Carolina.
• Work in a telephonic managed care setting.
• Facilitate a plan of care for high-risk member population.
• Assess physical, functional, psychological, and financial needs of members.
• Develop individualized care plans.
• Monitor care plans and ensure effective provision of services.
• Provide education to members regarding their condition and treatment.
• Participate in interdisciplinary rounds with healthcare team members.
• Must have a current RN license and clinical experience in NICU.

Requirements

• Registered Nurse (RN) with a current, unrestricted license.
• Experience in a Case Management role preferred.
• Three or more years of clinical experience in a Level III or above NICU.
• CCM certification required.
• Excellent communication and organizational skills.
• Must be self-motivated and able to prioritize tasks.
• Experience with data entry into a database or EHR.
• Commitment to excellence in customer service.
