Licensed Clinical Care Manager (CCM) - Remote

Overview

The Licensed Clinical Care Manager (CCM) is a critical part of our care team at Ophelia. As an Advanced Practice Practitioner, you will provide clinical care to stable patients receiving medication for opioid use disorder and a narrow suite of psych disorders (depression, anxiety, etc…).

In Short

• Join Ophelia to improve patient access to treatment for opioid use disorder.
• Work as part of a team including prescribing clinicians and nurses.
• Provide compassionate, evidence-based treatment to patients.
• Conduct follow-up visits and check-in calls with patients.
• Act as a liaison and advocate for patients.
• Escalate care needs to prescribing clinicians when necessary.
• No prior experience with medication for opioid use disorder required.
• Full-time position in a fast-paced startup environment.
• Support vulnerable populations through telehealth.
• Receive training on necessary skills for the role.

Requirements

• Must be a Nurse Practitioner or Physician Assistant.
• Passion for serving vulnerable populations.
• Interest in treating patients with opioid use disorder.
• Ability to work in a supportive team environment.
• Willingness to learn and adapt in a fast-paced setting.
