Overview

We are seeking a Licensed Clinical Social Worker who will work remotely in Connecticut, supporting our Medicare, Marketplace, and DSNP Population.

In Short

• Licensed Clinical Social Worker (LCSW) position.
• Remote work supporting Medicare and DSNP populations.
• Conduct telephonic assessments and create care plans.
• Participate in interdisciplinary care team meetings.
• Excellent computer skills required for multitasking.
• Fast-paced environment with productivity standards.
• Schedule: Monday to Friday, 8 AM to 5 PM EST.
• Local travel of up to 40% may be required.
• Must be licensed in Connecticut.
• Home office with high-speed internet required.

Requirements

• Graduate from an accredited nursing or social work program.
• Active behavioral health license required.
• 2+ years in case management or related fields.
• Experience with severe mental health concerns.
• Excellent communication skills.
• Data entry skills and experience with clinical platforms.
• Knowledge of behavioral health care principles.
• Experience managing chronic health conditions.
• Proficient in Microsoft Office applications.
• Valid driver’s license required.
