Overview

The Value-based Pay and Population Health Program Manager oversees and optimizes OCHIN’s systems, processes, initiatives, and tools to support the success of our members, focusing on equitable clinical, financial, and operational outcomes for OCHIN members.

• Lead the full lifecycle of strategic programs from scoping to execution.
• Build and maintain relationships with internal teams and external partners.
• Apply advanced project management techniques to ensure program governance.
• Use data and analytics to guide program direction and improve deliverables.

• Manage grant compliance and foster positive funder relationships.
• Act as the program’s voice to ensure stakeholder communication.
• Ensure alignment across teams to drive successful outcomes.
• Identify opportunities to optimize tools, technologies, and processes.
• Other duties as assigned.

Requirements

• Bachelor's degree required, master's degree preferred.
• Minimum 5 years' experience in program/project management required.
• Minimum 3 years of Epic Ambulatory and/or Healthy Planet experience preferred.
• Healthcare Payer experience preferred.
• PMP certification is a plus.
• Strong leadership and team management capabilities.
• Excellent communication and interpersonal skills.
• Strong analytical and problem-solving skills.
• Experience in budgeting and financial management.
• Demonstrated success in stakeholder engagement.
