An exciting opportunity has arisen for an enthusiastic Assistant Practitioner as a secondment opportunity to join the Eastern Home First Team, providing timely follow-up assessments for those discharged from acute hospitals.

Requirements

• Commitment to community health care
• Ability to work autonomously
• Experience in social work or related field
