Overview

The Field UAS Assessment Nurse conducts assessments to support care management within specified territories, ensuring members' needs are met through individualized care plans.

In Short

• Conduct UAS Assessments for care management.
• Responsible for 13-15 cases per week.
• Face-to-face assessments of members' needs.
• Develop Individualized Care Plans (ICP).
• Manage care transitions and discharge planning.
• Collaborate with the Interdisciplinary Care Team.
• Analyze data to enhance care delivery.
• Provide backup to other Assessment nurses.
• Willingness to travel for field visits.
• Training provided for recent graduates.

Requirements

• UAS Certified RN licensed in NY.
• Recent graduate with NY State RN License.
• Bachelor's Degree or Health Education.
• Strong clinical assessment skills.
• Excellent communication skills.
• Proficiency in Microsoft Office.
• Ability to work independently.
• Strong problem-solving skills.
• Highly organized and accountable.
• Collaboration and relationship-building skills.
