Community Health Assessment Registered Nurse - Remote

Overview

This position is responsible for completing Community Health Assessments (CHA) and related documentation based on New York State requirements, supporting the Senior Whole Health business.

In Short

• Complete CHA and enrollment paperwork for Medicaid and Medicare eligible members.
• Conduct assessments and develop care plans for members.
• Ensure accuracy and completeness of data collected.
• Attend training sessions for proper documentation.
• Support quality improvement initiatives.
• Must be knowledgeable in NY UAS and care management principles.
• Strong organizational and time management skills required.
• Travel to various locations in New York is necessary.
• Bilingual candidates are encouraged to apply.
• Work hours are Monday to Friday, 8:00 am to 5:00 pm EST.

Requirements

• Associates degree in Nursing required.
• Minimum two years of clinical experience in managed care.
• Experience with UAS and home visits is essential.
• Registered Nurse (RN) licensure required.
• Must reside in NY or neighboring states.
• Preferred: Bachelor’s degree in Nursing.
• Preferred: CCM or CCP certification.
• Experience in home care and long-term care is a plus.
• Basic computer skills are necessary.
• Ability to travel 50% of the time.
