Overview

The Rapid Rehousing Case Manager is a grant-funded position focused on providing case management and rental assistance to homeless individuals, families, and youth in Marion and Polk Counties.

In Short

  • Provide case management and rental assistance to homeless individuals.
  • Conduct eligibility screening and program enrollment.
  • Perform participant follow-ups and check-ins.
  • Process required paperwork and data entry.
  • Align services with Housing First initiatives.
  • Facilitate distribution of housing assistance funds.
  • Develop Individual Service Plans for clients.
  • Maintain relationships with community partners.
  • Ensure compliance with federal and state regulations.
  • Attend required trainings and maintain data quality.

Requirements

  • High School Diploma or GED with two years of social service experience.
  • College degree or specialized training preferred.
  • Knowledge of coordinated entry process and homeless service delivery.
  • Valid driver license and insured vehicle required.
  • Must pass background and drug screenings.
  • Basic computer skills and proficiency in MS Office.
  • Excellent planning, organization, and communication skills.
  • Training in trauma-informed care and related areas preferred.
