Overview

The Transitions of Care (TOC) Care Manager provides care management on behalf of the ACO’s medical homes, ensuring safe transitions of care from inpatient to outpatient settings.

Requirements

• Bachelor of Science in Nursing or Master’s Degree in Social Work.
• Current state licensure as RN, LSW, or LCSW.
• 3-5 years of experience in care management or public health.
• Knowledge of population health management systems.
• Excellent communication and organizational skills.
• Ability to work independently and in a team.
• Experience with electronic health records and care management platforms.
• Program development and training experience preferred.
• Bilingual in Spanish preferred.
