Overview

A dynamic role within the Transitions Planning Team, focusing on aiding young adults in their transition to adulthood and supporting clients aged 18-25 in accessing mental health services.

In Short

• Building therapeutic relationships with clients using a person-centered recovery approach.
• Providing psychologically informed interventions.

Requirements

• Experience working with young people or adults with care and support needs.
• A person-centered and strengths-based approach to practice.
• Previous experience with young people with learning disabilities.
• Familiarity with relevant legislation.
