Overview

We are seeking CA licensed Nurse Care Managers to join our Care and Case Management team, focusing on holistic member care throughout their healthcare journeys.

In Short

• Deliver coordinated, patient-centered virtual Care Management by telephone or video.
• Create impactful care plans with members and a diverse care team.
• Help members navigate complex medical conditions and the healthcare system.
• Provide compassionate, longitudinal follow-up care.
• Assist throughout acute healthcare episodes, providing coordinated Case Management.
• Coordinate resources addressing both clinical and social problems.
• Must have a Bachelor of Science in Nursing and active CA license.
• 5+ years of nursing experience with 2+ years in care management.
• Experience working remotely and with multiple medical record systems.
• Empathetic approach to patient care.

Requirements

• Bachelor of Science in Nursing (BSN).
• 5+ years of experience in nursing.
• 2+ years of experience in care management.
• Must have an active CA license and active CCM certification.
• Willingness to maintain licensure in multiple states.
• Work until 9-6 PM PST.
• Spanish speaking is desirable.
• Experience working remotely.
• Ability to communicate effectively with patients and families.
• Follow HIPAA regulations strictly.
