CA Licensed Nurse Care Manager, Adult - Remote

Overview

We are seeking California Licensed Nurse Care Managers to join our Care and Case Management team, focused on providing holistic care to members through their healthcare journeys.

In Short

• Deliver coordinated, patient-centered virtual Care Management by telephone or video.
• Create impactful care plans with members and the care team.
• Help members navigate complex medical conditions and the healthcare system.
• Partner with local providers to ensure coordinated care.
• Provide compassionate, longitudinal follow-up care.
• Assist during acute healthcare episodes and provide coordinated Case Management.
• Coordinate resources to address clinical or social problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Must have current CCM Certification or eligibility for exam in August 2025.
• 5+ years of experience in nursing.
• 2+ years of experience in care management.
• Must reside in a compact NLC state with an active California Nursing License.
• Willingness to maintain licensure in multiple states.
• Experience working remotely and using multiple computer/medical record systems.
• Empathy and ability to communicate effectively with patients and families.
• Ability to write medical information in easy-to-understand language.
