Licensed Nurse Care Manager, Adult (Remote)

Overview

We are seeking California Based Nurse Care Managers for our Care and Case Management team, who will guide members through complex medical and behavioral health situations, ensuring their needs are met with industry-leading interventions.

In Short

• Deliver coordinated, patient-centered virtual Care Management by telephone or video.
• Create impactful care plans with members and the care team.
• Help members navigate complex medical conditions and benefits.
• Partner with local providers for coordinated care.
• Provide compassionate, longitudinal follow-up care.
• Assist throughout acute healthcare episodes.
• Coordinate necessary resources to address members' problems.

Requirements

• Bachelor of Science in Nursing (BSN).
• Current CCM Certification or eligibility for exam.
• 5+ years of experience in nursing.
• 2+ years of experience in care management.
• Active California Nursing License.
• Willingness to maintain licensure in multiple states.
• Comfortable discussing several medical conditions.
• Experience working remotely and using medical record systems.
• Empathetic approach to patient care.
• Efficient in writing medical information in patient-centric language.
